29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  Location: 125 Nashua Road, Billerica, MA  On or about 9/17/13, workers were exposed to fall hazards of greater than 29-feet due to the side-rails of the ladder placed un-evenly and un-secured to the pitched roof of the two-story house.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Location: 125 Nashua Road, Billerica, MA  On or about 9/17/13, workers were exposed to fall hazards of greater than 29-feet while working from an 8-pitch roof without a fall protection system.
