29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts: On or about February 6, 2014, and times prior to, at 2300 145th Street in Little Rock, Arkansas, in the Aluminum Assembly area, the chain guard for the left side of the 10" bi-metal cutting blade on the Sears/Craftsman radial arm saw used for cutting aluminum was not positioned to prevent contact with the blade and no guard was provided on the right side of the bi-metal cutting blade. Employees were exposed to the hazard of contact with the rotating cutting blade.

29 CFR 1910.303(g)(1)(iii): At least one entrance of sufficient area shall be provided to give access to the working space about electric equipment: On or about February 6, 2014, at 2300 145th Street in Little Rock, Arkansas, the disconnect panel located on the northeast corner wall adjacent to the maintenance shop storage area was blocked by items such as but not limited to a wheel barrel, wooden boards and aluminum poles. This condition impeded direct access to the panel.
