29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating, or moving parts of equipment exposed to contact by the employees or which otherwise created a hazard, were not guarded: On or about February 6, 2014 the employer did not ensure that the flywheel housing on a motor used to power a cement mixer was guarded. This condition exposed employees to entanglement and contact hazards.
