29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a.  On or about Feburary 6, 2014, on the west side of building H, employees working near the edge were not protected from a 18 feet fall hazard by the use of a fall protection system.    b.  On or about Feburary 6, 2014, on the west side of building H, an employee working on a flat roof near the edge was not protected from a 18 feet fall hazard by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    a.  On or about February 6, 2014, on the east side of building H, the ladder used to access the roof did not extend 3 feet above the landing, exposing employees to a 18 feet fall hazard.
