29 CFR 1926.453(b)(2)(v): A body belt/harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about 02/05/2014 at the job site, no personal protective equipment was provided to the employees who were removing drip edge from the second floor landing while working from a Genie S-60 aerial lift serial #51255 exposing the workers to a 10 ft. fall hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about 02/05/2014 at the job site, drip edge from the floor landing was being removed on the second floor balcon. The use of fall protective equipment was not enforce exposing employees a 15 ft. fall hazard.
