29 CFR 1910.178(p)(1): Powered industrial trucks found to be in need of repair, defective, or in any way unsafe, were not taken out of service until they had been restored to safe operating condition: On or about January 29, 2014, employees were exposed to serious struck by injuries from the Tailift Forklift that was in operation, in that the brake pad was missing the slip resistant rubber piece.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about January 29, 2014, a 220V flexible cable had pulled loose from its attachment point to a fixed outlet box that was connected to a Vertical Mill leaving the cable attached to the device by its internal conductors.
