29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. (a) Roof trusses, All Saints Anglican Church located at 149 Ebenezer Road, Fayetteville, GA 30214: Employees working from a job-made scaffold platform engaged in roof truss framing activities at heights of approximately 23-feet, 6-inches were not provided with a fall protection system. Employees were wearing full body harnesses but failed to connect to the life-lines provided.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself. a) Roof trusses, All Saints Anglican Church located at 149 Ebenezer Road, Fayetteville, GA 30214: Employees failed to make compatible connections to the 5000 lbs. anchor point provided by the manufacturer. The snaphook was tied onto the 5/8" polyolefin life-line instead of being connected directly to the tempered anchor provided by the manufacturer which may compromise the equipment in the event of a fall exposing employees to potential injuries.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill. a) Roof Trusses, All Saints Anglican Church located at 149 Ebenezer Road, Fayetteville, GA 301214: Employees failed to install the Guardian Fall Protection Kit according to the manufacturer's requirements which compromised the equipment's effectiveness. The employer failed to ensure that each employee has been trained by a Competent or Qualified Person in the nature off fall hazards in the work area, the correct erecting, maintaining, dissembling and inspection of the fall protection system being used.
