29 CFR 1926.150(c)(1)(i): A fire extinguisher, rated not less than 2A, was not provided for each 3,000 square feet of the protected building area, or major fraction thereof:   a) First Floor of the Building - Fire extinguishers were not provided on each floor of the building including on the 1st floor where a diesel All-Pro Series 170T heater was in use exposing the carpenters to a potential fire hazard, on or about 02/06/14.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross-braces were used as a means of access:   a)  2nd Floor South Wall of the Building - Carpenters were observed standing on carpenter pole scaffolds which were not provided with ladder access exposing them to 6 foot fall hazard, on or about 02/06/14.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:   a) First Floor of the Building - Carpenters were observed standing on carpenter pole scaffolds which were not protected with fall protection while leaning over the exterior wall hoisting roof trusses exposing them to a 28 foot fall hazard, on or about 02/06/14.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
