29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: a) On or about February 6, 2014, the employer failed to ensure working surfaces inside the pad print room were maintained in a clean condition. Employees working in the pad print room were exposed to dermal contact with isocyanates in that the scale used in measuring the weights of ink tested positive for aromatic isocyanates after a surface SWYPE test.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) On or about March 3, 2014 the employer failed to train employees on the hazards of the chemicals they were working with. Employees working in the Pad Print Room were exposed to hazardous chemicals such as but not limited to toluene diisocyanate that is present in the C1 hardener.

29 CFR 1910.1200(f)(10): The employer did not ensure that workplace labels or other forms of warnings were legible. a) On or about February 6, 2014, the employer failed to ensure that containers were properly labeled with identity of the chemical. Acetone was stored in a plastic bottle and the chemical name was illegible.
