29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent):  a) for the work area where the employee stands to manually feeding polyvinyl chloride molding resin to the molding machines at cold blending area.  On or about 02/06/2014, an employee was observed standing at the open- sided platform exposing him to a 5-foot fall hazard.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a) for employees providing maintenance and repair services to the PCM mixer and or other extrusion molding machines as observed on or about 02/06/2014.  Procedures/policies were not established to control the accidental release of energy.
