29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:  a) Machine Shop - Employee used a Do All band saw, #21, that did not have a complete guard in place, thereby exposing the employee to a serious hazard, on or about 2-4-14.  Abatement certification required.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a) Welding Shop - Employees use a Baldor pedestal grinder, model 7307, 1/2 hp, to sharpen tungsten welding rods.  The grinding wheel has a work rest that was adjusted to a 1/2" opening, thereby exposing the employees to a hazard, observed on or about 2-4-14.  Abatement certification required.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch  (a) Welding Shop - Employees use a Baldor pedestal grinder, model 7307, 1/2 hp, to sharpen tungsten welding rods.  The grinding wheel has no safety guard in place so it can't be adjusted to a one fouth inch gap, thereby exposing the employees to a hazard, observed on or about 2-4-14.  Abatement certification required.
