29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.   On or about February 5, 2014, employee was removing a sign from a window and was operating a boom truck aerial lift. Employee was not wearing any type of fall protection. This condition exposed employee to falls greater than 4 feet.
