On or about February 7, 2014, the employer at Gandy Printers Inc., an employee (operators/mechanics) reached into the Rolan Mabeg Six Ink Printer machine to replace rollers without utilizing a locking device, exposing employees to crushing and amputations from the rotating drums.      c.  On or about February 7, 2014, the employer at Gandy Printers Inc., an employee (operators/mechanics) reached into the Thermo-O-Type Thermographer to make adjustments without utilizing a locking device, exposing employees to the pinches, burns and amputations from the conveyor.
