29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. on or about February 07, 2014, and times prior thereto, employees were using an energized dock light fixture at dock #40 in the shipping area that had damaged strain relief. The outer jacket was damaged and torn, exposing internal conductors to damage, and exposing employees to electrical shock and fire hazards.
