29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for removal and transfer of lockout devices or tagout devices and the responsibility for them:    On or about February 6, 2014, employees were exposed to struck-by and electrical hazards when the machine specific lockout procedures did not include specific instructions for lock removal.  Employees perform maintenance on equipment such as, but not limited to, M-300A-Harris infeed, Stahl Folder, M-300A-Print Unit, St-400 Stitcher, A Baler Diverter, and Harris M-300A.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about February 6, 2014, employees were exposed to chemical hazards when the employer had not updated the Hazard Communication program or provided the training on the new Hazard Communication standard changes.  Employees used chemicals such as, but not limited to, ABC Wash equipment cleaner, Novavit ink, and propane.
