29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:    On or about February 10, 2014 and times prior thereto, employees of Pine Bluff Crating & Pallet, Inc. manufactured wood pallets in the facility located at or near 2600 South Persimmon Street in Pine Bluff, Arkansas.  Employees operated a liquefied petroleum forklift manufactured by Hyster for transporting pallets.  The seat of the forklift had been modified with the replacement with a seat from an automobile.  The seat installed had not been approved by the manufacturer for use and did not have a seatbelt, exposing employees to ejection hazards.

Persimmon Street in Pine Bluff, Arkansas.  The employees operated a Newman KM-16 trim saws which was observed in the Back Mill area.  The Newman KM-16 consisted of a total of five (5) attached vertical trim saws which were not guarded, exposing employees to flying debris.

The flexible cord connected to the drill press had not been visibly inspected before use for external defects and had insulation damage visible, exposing employees to electrical hazards.

Persimmon Street in Pine Bluff, Arkansas.  Flexible cords were not connected with fittings so that strain relief was provided in the following instances:  a.  Employees operated resaw #1 located in the Back Mill Area.  Conductors were visible where the sheathing on the flexible cord had pulled from the motor.  b.  Employees operated a vertical table saw located in the Back Double Area.  The sheathing on the flexible cord had pulled from the attached control box, exposing conductors.
