The affected employee was fit tested using irritant smoke, Allegro Smoke Tubes for Qualitative Fit Tests (P/N 2050-01), but the respirator was not equipped with high efficiency particulate air (HEPA) or P100 series filters. The affected employee wore the N-95 respirator when an exhaust hood was pressure washed with a diluted mixture of Caustic Potash Liquid 45% (potassium hydroxide), Glycol Ether EB (2-butoxyethanol), and Surfonic N-95 (nonylphenol, ethoxylated), which is generally referred to as "bug juice", at Alden-Debes Rehabilitation and Health Care Center in Rockford, Illinois.

Affected employees were required to wear a Cordova NX95V Particulate Respirator, which was selected without knowing the airborne concentration of flour generated during this cleaning activity. b) On or about October 25, 2013, Phoenix Industrial Cleaning, Inc. did not identify and evaluate the respiratory hazard(s) prior to employees entering into flour silos #1, #2, and #3 to perform cleaning at Aryzta LLC (formally Otis Spunkmeyer, LLC) in Northlake, Illinois. Affected employees were required to wear a North 5500 half-face air-purifying respirator equipped with organic vapor/P100 cartridges, which was selected without knowing the airborne concentrations of the above chemicals generated during this spray painting activity.
