29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:     a)      On and or about 02-10-14, the employer was using a four (4) inch metal receptacle box as an electrical power tap to power the #22 DeWalt radial arm saw; the receptacle box was not listed and labeled to be laid on the floor.        b)      On and or about 02-10-14, the employer was using a four (4) inch metal receptacle box as an electrical power tap to power a green colored drill press; the receptacle box was not listed and labeled to be laid on the floor.        c)      On and or about 02-10-14, the employer was using a four (4) inch metal receptacle box as an electrical power tap to power a DeWalt 1030 radial arm saw; the receptacle box was not listed and labeled to be laid on the floor.        d)      On and or about 02-10-14, the employer was using a four (4) inch metal receptacle box as an electrical power tap as an extension of power outlets near the Production Shop electrical panels; the receptacle box was not listed and labeled to be laid on the floor.        e)      On and or about 02-10-14, the employer was using a four (4) inch metal receptacle box as the electrical power feed into a second four (4) inch metal receptacle box near the #22 DeWalt radial arm saw; the receptacle box was not listed and labeled to be laid on the floor.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  a)   On and or about 02-10-14, employees were using flexible cord which was positioned on the floor between a wall and equipment work bench as the electrical branch circuit to power stationary wood working equipment in the Production Shop.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:    a)  On and or about 02-10-14, employees had blocked two side by side electric panels in the Production Shop with a plastic debris tub, Clarke welder and helmet, and push broom/squeegee handle stored within five (5) inches of the panel fronts and with a stack of wood materials stored on the floor within two (2) inches of the stored equipment.
