29 CFR 1910.24(h): Standard railing(s) were not provided on the open side(s) of all fixed industrial stairway(s) and stair platform(s): a) On or about February 10, 2014, at the above located workplace, employees working on the sorting platform were required to use a stairway with five risers. The stairway did not have railings on its open sides, thereby exposing employees to fall hazards. The stairway was located on the east side of the conveyor belt.

There were no exit signs marking the west exit. 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees a hazard created by nip points and rotating parts: a) On or about February 10, 2014, at the above located work place, employees were assigned to work with and around a conveyor belt that was above a passageway and was less than 6 feet above floor level. The underside of the belt with nip points and rotating parts was not guarded, thereby exposing employees to laceration and contusions.
