Section 5(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm in that employees were exposed to being struck by a falling Mud Gas Separator, or "gas buster," during rig-down activities:    (a) On or about December 14, 2013, for the employees rigging down where one employee was severely injured after being struck by a falling gas buster while working under the load before the gas buster was properly restrained by a crane, and the pin had been removed at H&P Rig 457, six miles north of Highway 50 on County Road 7 North, near Powers Lake, North Dakota.    Abatement Note:  Among other methods, one feasible and acceptable method to correct this hazard would be to ensure that employees are not working under the load and/or pins are not pulled until the load is restrained at the top of the gas buster by crane.
