29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 6 February 2014, at the above addressed jobsite, two employees were sanding the wood frame around a window on the roof of a residential home under construction without the use of fall protection, exposing the employees to a fall hazard of approximately 28 feet in height to the ground below.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   On or about February 6, 2014 at the above addressed jobsite, the employer did not provide the information contained in the appendix D to employees using a 3M half-face respirator, exposing the employees to a health hazard.
