29 CFR 1910.1025(l)(1)(i): Employee(s) working in an area where there is potential exposure to airborne lead at any level were not informed of the content of Appendices A and B of 29 CFR 1910.1025:  On or about February 11th, 2014, at the above address, employees cleaning the shooting range were not informed on the contents of the appendices A & B.  The Pembroke Gun & Range LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.1025(l)(1)(i), which was contained in OSHA inspection number 313105637, citation number 1, item number 9 and was affirmed as a final order on 3/24/2010, with respect to a workplace located at 3130 SW 19th St Pembroke Park, Fl 33009.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   On or about February 11th, 2014, at the above address, the employer did not provide the information contained in the appendix D to employees using a 3M large half-face respirator during the cleanup of the shooting range.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:   On or about February 11th, 2014, at the above address, the employer did not provide a medical evaluation nor fit testing to employees using a 3M large half-face respirator during the cleanup of the shooting range.
