National Plastics, Inc. did not have specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy for machines with multiple energy sources. Two thermal forming machines, serial #s 07-04396 & 5498 located in the plant did not have specific procedural steps for locking/tagging them out.
