29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):  On or about 02/11/14, on the north side wall of the facility, the employer allowed employees to use a walkway adjacent to an inadequately guarded fuse box.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about 02/11/14, at the facility located at 8650 Box Rd in Shreveport, LA, the employer did not provide training to employees regarding the new label elements and safety data sheet format.
