29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   In the Welding Bay, On or about February 12, 2014, employees were exposed to amputation hazards when using a Metal Devil, (metal cutting) chop saw, which had a blade guard that did not retract to the closing position after the cut, while employees were cutting rectangular tubes.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:  In the Foam Bay, On or about February 12, 2014, employees were exposed to amputation hazards when using the Sterling bandsaw which did not have a guard on the unused portion of the saw blade, while employees were cutting small sections from a large foam piece.

29 CFR 1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:  In the Waterjet Bay, On or about February 12, 2014, employees were exposed to shock hazards when the access space in front of H2 electrical panel was blocked by a pallet with almandite garnet bags and scrap metal parts, while working on the air compressor and chilling unit.
