29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked between the front uprights and the guardrail supports:    a)  The employer erected two 12 ft tall scaffolds for installing a decorative metal curtain along the 25 ft tall wall (18th floor lobby).   The scaffold framings were 4 and 5 ft wide but the working platforms were only 20 inches wide, made of plywood nailed to 2by4 wood.  Employees were exposed to falling 12 feet.     Location: 125 Park Ave, New York, N.Y.    On or about 2/10/2014    NOTE: BECAUSE THE EMPLOYER CORRECTED THIS ITEM DURING THE INSPECTION THE EMPLOYER IS NOT REQUIRED TO SUBMIT ABATEMENT CERTIFICATION NOR DOCUMENTATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(f)(7): The employer did not ensure that scaffolds were erected, moved, dismantled, or altered only under the supervision and direction of a competent and qualified  person:    a) The employer erected two 12 ft. tall scaffolds for installing a decorative metal curtain along the 25 ft tall wall (18th floor lobby).  The scaffolds were not tall enough, did not have safety guardrails and fully planked platforms; employer used wood screwed together without tie wire to secure the connections. Employees were exposed to scaffold collapse hazard and fall hazard.      Location: 125 Park Ave, New York, N.Y.    On or about 2/10/2014    NOTE: BECAUSE THE EMPLOYER CORRECTED THIS CONDITION DURING THE INSPECTION THE EMPLOYER IS NOT REQUIRED TO SUBMIT ABATEMENT CERTIFICATION NOR DOCUMENTATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level. Paragraphs (g)(1)(i) though (vii) establish the types of fall protection to be provided to employees on each type of scaffold. Paragraph (g)(2) addresses fall protection for scaffold erectors and dismantlers:    a)  Employees were exposed to 12 ft fall hazard when working on the two 12 ft tall scaffolds in the 18th floor lobby without guardrails or personal fall protection systems (harness connected to a lifeline).
