1910.244(a)(2)(iii): The employer did not ensure that after a load was raised that it was cribbed, blocked, or otherwise secured: a) General Motors, LLC @ 1500 E. Route A, Wentzville, MO 63385: On November 1, 2013, employer did not ensure that the load was cribbed, blocked, or otherwise secured raised. Employees were tasked with lifting and repositioning a skuk, which was loaded with a van body, in the Body Shop at the lift table located at approximately GG-41. Employees utilized two, 2 ton, Omega Aluminum Racing hydraulic jacks positioned at opposite ends of the skuk to lift and sustain the load. Employees repositioned the skuk via pry bars and physical pushing while lifted by the hydraulic jacks. Employees attempted to lower the jacks in unison upon completion of the repositioning task when the skuk slipped from the saddle of the rear hydraulic jack and thereby caused a struck-by hazard to the employee who operated the rear hydraulic jack.

1910.244(a)(1)(i): The employer did not ensure that a jack that was used had a rating sufficient to lift and sustain a load: a) General Motors, LLC @ 1500 E. Route A, Wentzville, MO 63385: On November 1, 2013, the employer did not ensure that the Omega Aluminum Racing jacks had sufficient lift capacity to lift and sustain a skuk loaded with van body. Employees utilized two, 2 ton, hydraulic, Omega jacks to lift and reposition a skuk with a van body in the Body Shop at lift table located at approximately GG-41. The employer was unaware of the total weight of the skuk and van body.
