29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous.   On or about February 12, 2014, on the west side of the work site, employees engaged in roofing activities utilized an extension cord that was missing the ground pin exposing employees the hazard of electrical shock.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about February 12, 2014, on the west side of the construction site, employees engaged in roofing activities without fall protection were exposed to a fall hazard of approximately 20 feet to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about February 12, 2014, on the west side of the residential construction site, employees using an extension ladder to access the upper levels of the roof without the siderails extending at least three (3) feet above the upper landing were exposed to a fall hazard of more than 20 feet.
