29 CFR 1926.307(d)(1): Pulley(s) with part(s) seven feet (2.128 m) or less from the floor or work platform were not guarded in accordance with the requirements specified in paragraphs  (m) and  (o) of this section:Note:  Pulleys serving as balance wheels (e.g., punch presses) on which the point of contact between belt and pulley is more than 6 ft. 6 in. (1.976 m) from the floor or platform may be guarded with a disk covering the spokes.    On or about February 12, 2014, on the west side of the construction site, employees utilized an air-compressor with an unguarded pulley exposing employees to caught-by hazards.

On or about February 12, 2014, on the west side of the construction site, employees utilized an air-compressor with an unguarded belt exposing employees to caught-by hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).    On or about February 12, 2014, on the west side of the construction site, employees who worked without fall protection were exposed to a fall hazard of approximately 20 feet while engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.    On or about February 12, 2014, on the west side of the residential home, employees utilized an extension ladder to access the roof and the side rails did not extend 3 feet above the landing exposing the employees to a fall hazard of approximately 8 feet to the ground below.
