29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years:    On or about February 12, 2014 and times prior thereto, employees of American Tool & Machining Co., Inc. operated powered industrial vehicles throughout the facility located at or near 1910 Benton Street in Searcy, Arkansas.  The employer had not performed an evaluation of each operator's performance at least once every three years, exposing employees to struck-by, crushed-by, and tip-over hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about February 12, 2014 and times prior thereto, employees of American Tool & Machining Co., Inc. manufactured production and drilling parts for the oil and gas industry at the facility located at or near 1910 Benton Street in Searcy, Arkansas.  Machinery was observed unguarded exposing employees to rotating parts and flying debris in the following instances:    a.  A lathe manufactured by Yuasa with model number FX-4080 was observed with an unguarded chuck.    b. A lathe manufactured by Colchester with model number Clausing 21 and serial number MF0116 was observed with an unguarded chuck.    c. A milling machine manufactured by Bridgeport with serial number 173617 was observed unguarded at the point of operation.    d. A milling machine manufactured by Bridgeport with serial number 173604 was observed unguarded at the point of operation.    e. A milling machine manufactured by Bridgeport with serial number 130309 was observed unguarded at the point of operation.    f. A milling machine manufactured by Bridgeport with serial number 854252 was observed unguarded at the point of operation.    g. A milling machine manufactured by Bridgeport with serial number 166624 was observed unguarded at the point of operation.    h. A milling machine manufactured by Bridgeport with serial number 166613 was observed unguarded at the point of operation.    i. A milling machine manufactured by Bridgeport with serial number 152517 was observed unguarded at the point of operation.    j. A milling machine manufactured by Bridgeport with serial number 133559 was observed unguarded at the point of operation.

29 CFR 1910.213(i)(1): Non-working portion(s) of the blade of bandsaw(s) were not enclosed or guarded:    On or about February 12, 2014 and times prior thereto, employees of American Tool & Machining Co., Inc. manufactured production and drilling parts for the oil and gas industry at the facility located at or near 1910 Benton Street in Searcy, Arkansas.  Employees operated horizontal band saws in the cutting room which were not guarded in the following instances:    a. A horizontal band saw manufactured by Summit with model number 359-M and serial number 6-1978-2 was observed without guarding on the non-working portion of the blade.    b. A horizontal band saw manufactured by Johnson with model number J10 and serial number 184461 was observed without guarding on the non-working portion of the blade.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about February 12, 2014 and times prior thereto, employees of American Tool and Machining Co., Inc. performed maintenance and servicing on machinery with the use of lock out/tag out.  The employer had not established a program consisting of energy control procedures for the machines in the event of unexpected start-up.

On or about February 12, 2014 and times prior thereto, employees of American Tool & Machining Co., Inc. manufactured production and drilling parts for the oil and gas industry at the facility located at or near 1910 Benton Street in Searcy, Arkansas.  Lamp fixtures used for general illumination attached to machinery were not guarded in the following instances:  a.  An unguarded lamp was observed attached to the lathe manufactured by Colchester with model Clausing 21 and serial number MF0116 located in the machine shop.  b.  An unguarded lamp was observed attached to the Rob, Inc. band saw with serial number 5010 located in the cut room.
