29 CFR 1910.134(f)(2): Employee(s) using a tight-fitting facepiece respirator were not annually fit tested: (a) 55 Lower Hill side Road, Stevens Pennsylvania - An employee wore a half mask respirator while cleaning the smoke house. The employer did not ensure that the employee received an annual fit test, on or about 2/12/14. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1026(d)(1): The employer with a workplace or work operation covered by this standard did not determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI): (a) 55 Lower Hillside Road, Stevens Pennsylvania - An employee tig welded on stainless steel. The employer did not conduct monitoring to determine the employees 8 - hour time-weighted average exposure to chromium (VI), on or about February 12, 2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
