29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    (a) Production building: The use of safety glasses was not required in work areas where equipment such as a saw mill, planer, and a straight line machine were used. Flying particles of wood posed a hazard.

29 CFR 1910.134(c)(2): The employer did not develop and implement a respiratory protection program in accordance with sections i and ii of this paragraph (c)(2) for employees who chose to wear respirators where respirator use was not required:    (a) The employer provided N95 respirators for employee voluntary use. The following was not implemented:        - Employees were not provided the information in Appendix D of 1910.134.         - No assurance was made that respirators were cleaned, stored, and maintained properly.         - No action to establish the respirator user's medical ability to use a respirator was taken.
