29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems:  On or about February 13, 2014, Employer did not ensure guard rails were installed on a scaffold. Employees were plastering a wall on a scaffold without fall protection. This condition exposed employees to falls greater than 10 feet.

This condition exposed employees to falls greater than 10 feet.

On or about February 13, 2014, employer did not ensure to select an experienced and trained competent person to erect and dismantle the scaffold. This condition exposed employees to struck-by hazards in excess of 10 feet.
