29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    Stage 2 & 3 Screen Cleaning Room    a) An employee working as a screen cleaner was exposed to continuous noise levels at 121.7% of the permissible daily noise exposure (8 hour Time Weighted Average sound level of 90dBA) which is 1.21 times the allowable exposure limit.  The action level (AL) is 85 dBA.  The equivalent sound level of the measured dose is 91.4 dBA.  Sampling was performed for 438 minutes on 03/17/14 with the unsampled portion of time considered to represent zero for the exposure calculation.    b) An employee working as a screen cleaner was exposed to continuous noise levels at 74.39% of the permissible daily noise exposure (8 hour Time Weighted Average sound level of 90 dBA) which is 0.74 times the allowable exposure limit.  The action level (AL) is 85 dBA.  The equivalent sound level of the measured dose is 87.8 dBA.  Sampling was performed for 437 minutes on 03/17/14 with the unsampled portion of time considered to represent zero for the exposure calculation.

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user:    a) Facility    Employees cleaning screens are provided with 3M half mask, negative pressure respirators on a voluntary basis.  Employees using such respirators were not medically evaluated.  In addition, respirators were not cleaned, stored and maintained.    Violation occurred on or about 02/27/14.

29 CFR 1910.1048(d)(1)(i): Employees of a workplace covered by this standard were not monitored to determine their exposure to formaldehyde:    1) Facility    The employer did not monitor employees to determine their exposure to formaldehyde while mixing Fujifilm Sericol Texcharge Activator TC 747 (which contains 1.92% formaldehyde) with Fujifilm Sericol Texcharge TC Series Screen Ink and during the curing process after the ink is applied to the shirt during screen printing.  The mixture of the activator with the ink contains 0.09% formaldehyde.    Violation occurred on or about 02/27/14.
