29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) On or about March 11, 2014 employees were exposed to a tripping hazard in the maintenance due to the accumlation of trash, boxes and other equipment on the floor. b) MCCP - On or about March 11, 2014 employees were exposed to a tripping hazard due to two (2) three inch high spare conduits on the floor of Engine Room #1.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): a) MCCP Room - On or about March 11, 2014 a four hundred eigthy (480) three (3) phrase electrical panel lacked approved cover/enclousures exposing employees to the hazard of contacting an energized source.
