29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. a) Integrity Contracting & Consulting, LLC on or about February 13, 2014 at the address of 1050 Interstate Dr. Findlay, OH: The employer failed to ensure employees were protected from an approximant 33 ft. fall to the ground below while performing work covered under the scope of steel erection. No acceptable form of fall protection was in place while employee walked/worked on the 4rd floor's northeast material landing area (room 429).

29 CFR 1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of 29 CFR 1926.761: a) Integrity Contracting & Consulting, LLC on or about February 13, 2014 at the address of 1050 Interstate Dr. Findlay, OH: The employer did not ensure that employees had been trained in the recognition of fall hazards commonly created during steel erection activities. Employees unfamiliar with the requirements of fall protection during steel erection activities were exposed to falls up to 33 ft.
