29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite, 6424 Forward Avenue, Pittsburgh, PA: On or about February 14, 2014, 4 employees were working on a flat roof while they were removing a defective heating and air conditioning unit and replacing it with a new unit. The employees were not utiliizing any type all of fall protection and did not have warning lines in place. They were exposed to a fall hazard of approximately 60 feet to the ground.

29 CFR 1926.503(c)(3): Inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill: a) Jobsite, 6424 Forward Avenue, Pittsburgh, PA: On or about February 14, 2014, employees were working on a commercial roof without fall protection. The employer provided training documentation for the employees, but the knowledge of when it is required was not demonstrated while they were replacing a defective heating and air conditioning unit at heights of 60 feet above the ground.
