29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about February 14, 2014, at the above addressed job site, employees were exposed to the hazards of struck-by and amputation, while using horizontal band saw # H-12 that did not have safety guards while cutting metal pipes. b. On or about February 14, 2014, at the above addressed job site, employees were exposed to the hazards of caught-in, while using horizontal band saw # H-12 that had exposed rotating metal gears. c. On or about February 14, 2014, at the above addressed job site, employees were exposed to the hazards of struck-by and amputation, while using hydraulic power press # 4-4079 to bend metal components. d. On or about February 14, 2014, at the above addressed job site, employees were exposed to the hazard of amputation, while using hydraulic press brake machine # G1-110 that did not have safety guards while working with metal components.
