29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury:    On or about February 13, 2014 a full pallet, stacked 32 inches high of OSB was placed on roof trusses that had a maximum stack height limit of 16 inches of OSB.  Workers were not instructed as to the safe construction load limits and placement of OSB on.  This condition exposed employees to the hazards of falls and being struck by materials at the new building being constructed at 706 West Martin Luther King Blvd, Austin, TX 78705.  From BCSI-B4 Summary Sheet:  "WARNING Stacking excessive amounts of construction materials on floor or roof trusses is an unsafe practice.  Property damage, personal injury and/or death are possible if this warning is not heeded."

29 CFR 1926.501(a)(2): The employer did not determine if the walking /working surface on which its employees are to work had the requisite strength and structural integrity to support employees safely:    On or about February 13, 2014 a 32 inch stack of OSB 32 inches high was placed onto roof trusses that had a maximum load rating for a stack of 16 inches of OSB.  The excess weight caused the roof trusses fail thus causing structural failure of the roof where employees were working. This condition exposed the decking crew to a fall from elevation hazard and workers below to a struck by falling lumber, employees and tools hazard at the new building being constructed at 706 West Martin Luther King Blvd, Austin, TX 78705.
