OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with falling through a concrete form: Fabrication Area: On or about January 14, 2014, employees were exposed to hazards associateds with falling through a concrete form while working and walking on the formwork which was not designed and/or fabricated to support them. An employee was injured when a plywood filler gave way when he stepped on it.
