29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:                                                                                                                                           On or about February 14, 2014, at the workplace located at 3909 Clear Creek Court, Richardson, Texas; employees were exposed to fall hazards while working above 6-feet without the use of a guardrail system or personal fall arrest system.

29 CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided:     This violation was most recently observed on or about February 14, 2014, at the construction site located at 3909 Clear Creek Court, Richardson, Texas; where employees were exposed to fall hazards while not using a ladder where there was a break in elevation of more than 19-inches.
