Two subcontractor employees were working on the second level of the scaffold. This condition exposed the subcontractor employees to a fall of approximately 10.7 feet.

The second level of the scaffold was missing toprails, midrails, and endrails. This condition exposed the subcontractor employees to a fall of approximately 10.7 feet.

Scaffolds were not plumb, level, and squared. All brace connections were not secured: (a) MAJJAC INTERIORS, INC. at 2340 E Colfax, Denver, CO 80206: On February 11, 2014 the controlling employer did not ensure that the mobile scaffold was braced in that several cross, horizontal, and diagonal braces were not installed. The mobile scaffold was not plumb and connection pins were not installed. Two subcontractor employees were working on the second level of the mobile scaffold. This condition exposed the subcontractor employees to a fall of approximately 10.7 feet.
