29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:  (a)  On the east wall scaffold:  On February 18, 2014, the employer failed to provide a guard rail system along all open sides of the scaffold, exposing employees to falls of approximately 14 feet.

29 CFR 1926.452(c)(2): Tubular welded frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would have secured vertical members together laterally:  (a)  The east wall scaffold:  On February 18, 2014, the employer failed to brace the middle three sections of the scaffold as well as other sections, exposing the employees to struck-by hazards from a collapsing scaffold.
