29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: Steckline Communications, Inc., 1632 N. Maize Rd., Wichita, KS 67209 - On or about February 09, 2014, the employer failed to report to OSHA a work-related fatality within 8 hours of the incident. 9 CFR 1903.19(c)(1) abatement of the above violation was verified at time of inspection, no certification is required.
