29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded:  a)  Hangar 32;  Womens Restroom next to Seafood Inspection;  The floor hole measuring 4 inches in diameter by 4 inches deep was not covered, exposing employees to tripping hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)      Building 1, 2nd Floor, Finance cubicle by window;  A black heater was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.  CORRECTED DURING INSPECTION  b)      Building 1, 2nd Floor, Kitchen Area of CFO;  A white GE microwave was plugged into a brown extension cord.  An Oasis water cooler was not grounded and plugged into a black extension c ord.  A Hotpoint refrigerator was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.  c)       Building 1, 1st Floor, IT Work Area;  A Sanyo refrigerator and water cooler were plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.   d)      Building 1, 1st Floor, IT Cubicle Work Area;  Several relocatable power taps were plugged into  UPS surge protector units. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.   e)      Building 1, 1st Floor, IT Cubicle;  A New Air Heater was plugged into a relocatable power tap. The UL listing states that relocatable power taps are designed for low amperage computer and audiovisual types of equipment, and not appliances.  This exposes employees to possible electrical fire hazards.   f)       Building 1, 1st Floor, Real Property, Safety, Facilities, Logistics cubicle.  A white relocatable power tap was plugged into another white relocatable power tap. The UL listing states that relocatable power taps are designed to be plugged into an outlet, and not daisy chained and plugged into other relocatable power taps or extension cords.  This exposes employees to possible electrical fire hazards.   g)      Hangar 33; BMB Warehouse; An orange pigtail extension cord was powering a phone and HP Laserjet 2420 printer.  The UL listing states that extension cords shall only be used on a temporary basis.  This exposes employees to possible electrical fire hazards.    The National Oceanic and Atmospheric Administration was previously cited for a violation of this standard which was contained in OSHA inspection no.  906829 citation 2, item 4, issued on June 4, 2013 with respect to a workplace at Pasco, Washington.  The National Oceanic and Atmospheric Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 915685 citation 1, item 1, issued on July 26, 2013 with respect to a workplace at Long Beach, California.  The National Oceanic and Atmospheric Administration was previously cited for a violation of this standard which was contained in OSHA inspection no. 915443 citation 1, item 2, issued on August 9, 2013 with respect to a workplace at Long Beach, California.

29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent:  a)  Building 1, 2nd Floor, Corporate Services, Finance,  West side of building;  Exit signs were not placed over the entrance openings to the exit corridor.  b)  Building 1, 1st Floor, Corporate Services, CAO, Facility Logistics;  There was not an exit sign over the conference room door which lead directly to the exit corridor.  c)  Building 1, 1st Floor, Real Property, Safety, Facilities, Project Planning;  Exit signs were not placed over the entrance openings to the exit corridor.  d)  Building 8; BMB Warehouse, Inner and Outer Shipping and Receiving Office;  An exit sign was not placed over the door which lead to the nearest exit.
