29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about February 18, 2014, employees installing the coping cap on a commercial building were not wearing hard hats when working at lower levels exposing employees to being struck-by falling and flying objects from above.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level. On or about February 18, 2014, an employee working from a work platform elevated by a JLG 8042 tele-handler while engaged in installing flashing and coping cap on a commercial building was exposed to a fall hazard of approximately 30 feet to the concrete below.
