On 2/18/2012, Corrpak, Inc. had not developed, documented and utilized proper procedures for the control of potentially hazardous energy for two lathes and a milling machine located in the machining area of the plant.

29 CFR 1910.147(c)(4)(ii)(D):   The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  Coorpak, Inc. did not have specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.  The only lockout procedure available was for a screen roller, serial number B2417.  The procedure was missing the requirement to test the machine to determine and verify the effectiveness of the lockout.

One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  On 2/18/2014, employees from Corrpak, Inc. were exposed to hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  The following machines did not have guards:  Bridgeport Milling machine, serial # J17565 was missing covers to guard the belts and pulleys;  and a Guardian Drill Press, model # FDM-58-16S, was missing a guard to cover the bit.

29 CFR 1910.215(b)(9):   Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, were not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.  On 2/18/2014, employees from Corrpak, Inc. were exposed to struck-by hazards.   The tongue guards on a pedestal grinder, serial #200613-YL0006, and bench grinder, serial # 0002049, were not adjusted to one-fourth inch.
