29 CFR 1926.502(d)(6)(ii): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.     On or about 02/19/2014 at 2375 Sheridan Road, Decatur, GA. two employees tearing down an awning at the Shell Service Station were exposed to a fall of more than twelve feet to the lower level when their lanyards were wrapped around the H-beam and angle iron, that was part of the awning structure and the snaphook was connected back onto the lanyard.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.     On or about 02/19/2014 at 2375 Sheridan Road, Decatur, GA. an employee tearing down an awning at a Shell Service Station was exposed to a fall hazard of approximately twelve feet to the lower level when he was not protected by a guard rail system or personal fall protection.     The Shady Vent Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 315737981, citation number 01 item number 001 and was affirmed as a final order on 09/29/2011 with respect to a workplace located at 2755 Clairmont Road, Atlanta, GA.
