29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift  On or about June 30, 2016:  a) An employee was exposed to a fall hazard of approximately 10.5 feet while utilizing an aerial lift to assist in placing a u channel for a canopy repair, when the employers representative failed to ensure that the employee was wearing proper fall protection means.  No abatement certification or documentation is required for this item.  Shady Vent Constrcution, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(v)(ii)which was contained in OSHA inspection number 959007, citation number 2, item number 1 and was affirmed as a final order on April 24, 2014, with respect to a workplace located at 2374 Sheridan Road Atlanta, GA 30324 .
