29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about 9/19/2018 at 7342 Spout Springs Road Braselton, GA where the employer failed to ensure that workers   performing canopy installation activities were connected at all times.     a. A worker was observed exposed to 16-foot fall hazards to the concrete lower level while not connected.    b. A worker was observed connected and disconnected to remove frames from the forks of the forklift that were raised to the 16-foot working level.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    On or about 9/19/2018 at 7342 Spout Springs Road Braselton, GA where the employer failed to ensure that workers performing canopy installation activities were connected at all times. A worker was observed exposed to 16-foot fall hazards to the concrete lower level while not connected while in the JLG lift.    Shady Vent Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1159322, citation number 1, item number 1 and was affirmed as a final order on XXXXXXX, with respect to a workplace located at 4434 Cornelia Hwy  Lula, GA 30554.
